The Cosmetic Dentistry Grants Program

A Cosmetic Dentistry Grant (CDG Grant) is free to apply for and is available to anyone that wishes to improve their oral health and return confidence to their smile.

To be eligible for consideration of a CDG Grant, an applicant simply needs to demonstrate that their mouth is healthy enough to support the type of cosmetic dentistry procedure they wish to receive. To determine suitability, you are provided with a free oral health assessment(x-ray charges may apply) by a Certified Dental Practitioner in your area.

Following completion of your oral health assessment and when all basic dentistry is accomplished by the participating dentists, and presuming you are a good candidate for cosmetic procedure and implant, the dental practitioner will recommend you for inclusion in the CDG Program and provide you with a treatment plan that will help achieve your desired results.

Please note: The dentist will not compromise the function or health of your mouth to accommodate a cosmetic procedure. Accordingly, the dentist will advise in advance if you require any basic dentistry such as root canals, fillings, cleanings or extractions. If you do require any basic dentistry, you must complete it in advance of being recommended for consideration of a Cosmetic Dentistry Grant. Who Applies for a Cosmetic Dentistry Grant?

Cosmetic and implant dentistry has become a widely accepted way to achieve a healthy, confident smile and each year millions of North Americans from all socio-economic backgrounds share the same vision and goal of improving the appearance and function of their teeth with one or more cosmetic dentistry procedures.

Qualifying As a Candidate

To determine your eligibility, you will be provided with a free oral health assessment by a Certified Dental Practitioner in your area. A potential grant recipient’s residential address must be within a 60-mile radius from a participating CDG dentist. Following completion of your oral health assessment and completion of all basic dentistry, and presuming you are a good candidate for cosmetic and implant dentistry, the dental practitioner will recommend you for inclusion in the CDG Program and develop a Cosmetic treatment plan that will help achieve your desired results.

Awarding of Cosmetic Dentistry Grants

Partial Grants

Partial Grants are awarded to any qualified applicant on a per capita, percentile basis, distributed proportionally based on the number of applicants in the distribution pool, the value of their recommended treatment plan, and the amount of proceeds available for distribution.

Once becoming eligible for a partial grant, applicant’s must proceed with their cosmetic dentistry work with the dentist they were assigned to, or another participating dentist in the program. Should an applicant begin their cosmetic dentistry work with another dentist who is not a participating member of the CDG program, then CDG retains the right to revoke the partial grant from the applicant at any time.

Notification of awarding of a partial grant will be 30 to 60 days following an applicant’s completion of all basic dentistry and becoming recommended for inclusion in the grant program. Once acknowledgement and acceptance of the grant is received within the 30 day acceptance period, applicants are entitled to initiate their treatment immediately with their participating CDG dentist. Each applicant must submit a signed affidavit and thank you letter to the CDG office in order to confirm acceptance.

Payment of an applicant’s partial grant is made directly to the dental office and not to the applicant. It is the responsibility of the dental office to reimburse the patient for any money he or she has put forth towards their cosmetic dentistry work.

(Dentists may support the OAAG by purchasing advocacy programs, or may assist by contributing their professional services)


Allocations & Conditions for CDG Funding

To be eligible for consideration of a Cosmetic Dentistry Grant, you must receive a signed declaration form from the examining dentist confirming that you do not require any basic dentistry work such as fillings, cleaning, extractions or root canals. If basic dentistry is required, it must be completed at your own expense. Grant recipients must complete their cosmetic dentistry with the dentist that recommends them as a candidate for consideration of a cosmetic dentistry grant. Any dentist may apply for consideration to participate as a provider clinic provided he or she is:

  • located in the United States.
  • qualified to perform the recommended treatment.
  • in good standing with his or her respective governing body.
  • licensed as a recognized dental practitioner in the jurisdiction in which their practise is located.
  • willing to agree to the terms and conditions for participation and acceptance of the grant.
  • willing to take your case.

Up to $1 Million of net proceeds and resources will be allocated to grant applicants each year, distributed on a monthly basis.

Awarded recipients must begin their treatment plan within 3 months. There is no cost to apply, and you are not required to repay the cosmetic dentistry grant.

Your application remains eligible for up to 1 year and you may only apply one time.

How the Process Works

  1. An applicant submits a request for funding at our website.
  2. The applicant’s information is confirmed by our staff.
  3. A dentist near the applicant agrees to provide a free assessment and consultation , then contacts the applicant to arrange an appointment.
  4. The dentist determines eligibility. If the applicant does not qualify, the dentist will make necessary recommendations.
  5. If/when eligible, the dentist will provide the applicant with a cosmetic or implant treatment plan.
